Hello, All:

I started volunteering with KKFI over 4 years ago. I once saw Mike
Murphy use a form that was a half page of legalese. I didn't like it.
I lobbied to change the policy to allow the use of the creative commons
attribution share-alike (CC BY-SA) license. KKFI's legal counsel said
he loved the creative commons. KKFI does not have an official policy on
this as far as I know. I use the attached form, which invites the
person to specify either public domain or CC BY-SA (text copied below).

1. Creative Commons (CC) is a collection of licenses that support free
distribution. The “BY-SA” license authorizes redistribution under an
identical ("not more restrictive") license (share alike, SA) of
derivative work with attribution (BY). The official verbiage is
available on the web. Version 4.0 is international. The term “public
domain” does not mean the same in some countries as others; CC0
provides an international standard for what is know in the US as “public
domain”. The complete verbiage is available on the Internet.
